Our flagship 12-week transition programme
Twelve weeks. One individual plan. A purposeful balance of learning, confidence and practical preparation, shaped around the learner and the next chapter of their education.

Reconnect 12™
Reconnect 12 is for learners who need a supported bridge towards their next education or training step. That might mean rebuilding engagement, preparing to return to a setting or developing the confidence and skills to approach a new pathway.
We agree the learning priorities, timetable and support arrangements around the learner. Five Rs shape the twelve-week programme: Reconnect, Reengage, Rebuild, Relearn and Reach Forward. They bring together relationships, purposeful participation, confidence, teaching and preparation for the next step. The pace and balance of activity are agreed within the individual plan.
Explore the learning
Explore the five Rs of Reconnect 12. Together they provide the programme framework, with learning, support and review shaped around the individual. The pace, priorities and activities are agreed together, rather than assigning fixed weeks to each R.
Understand the person and agree a shared starting point.
Build a routine and a meaningful reason to take part.
Develop useful skills, confidence and self-belief.
Teach, practise and revisit agreed knowledge and skills.
Bring learning together and prepare a realistic next step.
01 / Reconnect
Listen to the learner’s story, strengths and hopes. Build a trusted relationship and make the first learning steps feel manageable.
02 / Reengage
Use the learner’s interests, a predictable routine and manageable tasks to build purposeful participation. Connect practical activity with goals that matter to them.
03 / Rebuild
Notice strengths, practise useful skills and recognise what is changing through real learning experiences. Help the learner experience success they can recognise.
04 / Relearn
Connect agreed academic priorities with interests and practical activity. Teach and revisit knowledge and skills, using feedback to choose the next challenge.
05 / Reach Forward
Explore the proposed next setting or pathway and the skills needed to approach it. Bring together learning, effective support strategies and learner voice so the final review leads to a practical handover and clear actions.
The five Rs guide the twelve-week programme. Individual priorities, pace, timetable, delivery and transition arrangements are agreed together; there are no fixed week allocations for each R.
Planning, support and review
Listening, planning and review support the whole programme. The six-week review is a chance to consider progress and agree adjustments; the final review brings together learning and agreed onward actions.
Understand the learner’s strengths, experiences and intended direction.
Confirm priorities, delivery, timetable and support arrangements.
Teach, capture progress and adapt through regular review.
Coordinate the final review, handover and agreed onward actions.
Progress you can understand
Success looks different for each learner. The programme brings together academic learning, participation, confidence and preparation so that progress is understood in context.
Reviews bring together the work, the learner’s voice and observations about the support they need. That picture helps families and referring professionals plan what comes next.
Clarity before the first session
The programme’s purpose, learning priorities and relevant adjustments are discussed around the learner.
Timetable, hours, setting, delivery format, costs and commissioning responsibilities are confirmed individually.
Named contacts, safeguarding arrangements, communication, reviews and transition responsibilities are clarified before provision starts.
A twelve-week framework does not imply a fixed number of teaching hours, a qualification or a guaranteed onward placement.
Before we begin
Start a conversation about the learner, the support you need and the practical arrangements.
Talk to our team →The programme develops skills and provides learning evidence; it does not automatically award a qualification. If an accredited route is proposed, the qualification title, level, awarding organisation, eligibility, assessment arrangements and fees will be confirmed before enrolment.
We discuss suitability, learning priorities, the delivery setting, timetable, fees, relevant access and support needs, and the responsibilities of the adults involved. Any qualification entry or external activity is confirmed separately.
The timetable, balance of subjects, delivery and costs are agreed for the individual programme. Twelve weeks describes the programme period, rather than a single standard package of hours.
Depending on the agreed plan, support can combine tuition, mentoring, practical projects, life skills, PSHE, online safety and preparation for the next setting. The scope is confirmed before delivery.
The programme prepares and supports a transition. Any school, college, training or employment arrangement must be agreed with the relevant provider; a destination is not guaranteed.
