Reconnect 12™ is Reconnect Learning's structured 12-week school re-engagement, reintegration and transition programme for children and young people experiencing persistent absence, EBSNA, SEND or SEMH barriers, exclusion risk, reduced timetables or disrupted education.

The programme gradually rebuilds trust, routine, confidence and participation before supporting a planned transition back to school, into a new education setting or towards another appropriate positive destination.

Why Reconnect 12™ Exists

Returning to school is not always as simple as walking back through the door

Persistent absence and school disengagement usually develop for a reason. Anxiety, unmet SEND needs, sensory difficulties, difficult relationships, trauma, care experience, academic gaps, exclusion and loss of confidence can all affect a learner's ability to attend.

After prolonged absence, even a learner who wants to return may find the idea of lessons, peers, routines and expectations overwhelming.

Reconnect 12™ creates a staged bridge between disengagement and meaningful participation, giving the learner time to rebuild before expectations increase.

Transition Back to School

Reintegration is planned with the learner — not done to them

Where returning to school is the agreed outcome, transition planning begins before the final week. Learner voice, school expectations, known triggers, reasonable adjustments and practical barriers all need to be considered.

The aim is a realistic and sustainable transition, rather than a rapid increase in attendance that cannot be maintained.

Reconnect 12™ is designed to support education re-engagement and preparation for reintegration. A successful return depends on the learner's needs, progress, circumstances, receiving setting, available adjustments and wider support network.

Where returning to the previous school is no longer appropriate, the final phase may instead support transition into another school, college, training, alternative provision or other agreed educational pathway.
